My clinical work is rooted in comprehensive geriatric medicine and focuses on delivering person-centred, coordinated care across settings. I provide services through Geriatrics outpatient clinics, Geriatrics inpatient care, and home-based medical care, working with multidisciplinary teams to support older people living with frailty, multimorbidity, cognitive impairment, and complex care needs. A core thread across all settings is helping patients and families navigate health and social care—aligning clinical decision-making with functional goals, quality of life, and the realities of everyday living.
Geriatrics Outpatient Services
In the outpatient setting, I offer comprehensive assessment and longitudinal follow-up for older adults with complex needs. Consultations typically integrate medical review with functional assessment, medication optimisation, cognitive and mood screening, falls risk evaluation, and goal-oriented care planning. Where relevant, I work closely with patients and family caregivers to clarify priorities, support shared decision-making, and coordinate referrals and community resources to strengthen continuity beyond the hospital.
Geriatrics Inpatient Services
Within the geriatric ward, I provide acute inpatient care for older adults with complex medical needs—addressing acute illnesses while taking account of frailty, multimorbidity, and functional vulnerability. Care is delivered through a coordinated, interdisciplinary approach, bringing together doctors, nurses, pharmacists, rehabilitation professionals, social workers, and dietitians to ensure comprehensive assessment and timely management. Alongside stabilising acute medical problems, our team focuses on preventing and managing geriatric syndromes (such as delirium, falls, deconditioning, and polypharmacy), optimising nutrition and function, and undertaking proactive discharge planning to facilitate safe transitions of care and to reduce avoidable complications and readmissions.
Home-based Medical Care
Through home-based medical care, I support older people whose mobility limitations, disability, or caregiving circumstances make clinic attendance difficult. Home visits enable assessment in the context of patients’ real living environments—often revealing practical barriers and strengths that are not visible in hospital-based care. This service focuses on stabilising chronic conditions, managing symptoms, reviewing medications, and coordinating care plans with family caregivers and community services, with the aim of helping patients remain safely at home and maintaining dignity and independence wherever possible.
